Support for Students with Disabilities
The faculty of Agriculture has established a system to support the study of students with disabilities through the Center for Health Science and Counseling Ito Center Zone Branch (Support Section for Inclusion), which serves as a consultation window for the entire university.
1. Consultation system
Students with disabilities consult with the Center for Health Science and Counseling Ito Center Zone Branch (Support Section for Inclusion) (hereinafter referred to as "IN Section"). The IN Section will then conduct an interview with the student. The interview with the student will be conducted by the "Interview Manager" and "Interview Respondent " (hereinafter referred to as "Interviewer").
2. Preparation of a written request for reasonable accommodation
After meeting with the interviewer, the student fills in the necessary items on the "Request for Reasonable Accommodations for Classes, Exams, and Daily Life (Form 1)" and the attached "List of Classes and Exam Subjects for which Reasonable Accommodations are Requested".
If the interviewer determines that the student requires reasonable accommodation for classes, examinations, daily life, etc. (hereinafter referred to as "accommodation"), the interviewer will confirm the student's wishes and intentions and indicate on Form 1 whether or not the request needs to be renewed each semester.
The interviewer shall refer to the medical certificate, disability certificate, results of psychological tests, etc., findings of specialists, and past records of consideration, etc., when considering the contents of consideration.
If it is difficult for the student himself/herself to prepare Form 1, the parent/guardian may prepare it on behalf of the student in consultation with the interviewer, etc. If it is difficult for the student and his/her guardian to prepare the Form 1, the interviewer may prepare the Form 1 and the attached sheets with the consent of the student and his/her guardian.
If the interviewer determines that an early meeting between the student and the faculty member in charge of the class is necessary, the interviewer may, with the consent of the student, provide the necessary information to the relevant department via the Student Support Division before the written request is submitted.
3. Submission of a written request for reasonable accommodation
Form 1 and Appendix (classes and examinations for which reasonable accommodation is requested) are to be submitted to the Student Support Division by the student him/herself.
If it is difficult for the student to submit the form in person, an interviewer, etc. will submit the form on behalf of the student. The interviewer shall provide the Student Support Division with the electronic data of Form 1 and the attached sheet.
4. Sending a Request for Reasonable Accommodation
The Student Support Division shall send the written request to the Student Affairs Division of the Faculty of Agriculture (hereinafter referred to as "Student Affairs Division"). The written request will be sent as an electronic file using the file-sharing system provided by the University.
5. Consultation on reasonable accommodation in the Faculty of Agriculture
The Student Affairs Division requests the Academic Affairs Committee of the Faculty of Agriculture to consider the details of reasonable accommodation. The Dean of the Faculty of Agriculture, upon receiving a report on the review results from the Academic Affairs Committee of the Faculty of Agriculture, determines the details of consideration.
6. Notification of the details of consideration
The Student Affairs Division will prepare a "Request for Reasonable Accommodation (Form 2)" in the name of the Dean. The Student Affairs Division will send this "Letter of Request for Reasonable Accommodation" to the faculty member in charge and send a copy of the "Letter of Request for Reasonable Accommodation" to the Student Support Division and the IN Section. In addition, the Student Affairs Division prepares a "Letter of Notification of Reasonable Accommodation Request (Form 3)" in the name of the dean and sends it to the student.
7. Constructive Dialogue
The constructive dialogue to discuss the details of how to implement reasonable accommodation is conducted by notifying the student of the faculty member's response to Form 2 and the results of the dialogue (constructive dialogue in writing).
The faculty member in charge of the student will examine whether the requested consideration can be implemented based on the Sample Form 2 sent from the Student Affairs Division, fill in the response to the Sample Form 2, and send it to the Student Affairs Division. If the faculty member determines that "consideration is required" or "implementation is not possible," the reason for the determination should be stated. The Student Affairs Division will send Form 2 with the results of the faculty member's review to the student and the Student Support Division/IN Section as needed.
Based on Sample Form 2 with the results of the faculty's review, the student confirms the reason for the requested consideration being "consideration required" or "not feasible," and if constructive dialogue is still required, a constructive dialogue (discussion based on the educational objectives and methods of each course) is held in person to reach a consensus through mutual understanding and decision-making. If a constructive dialogue is still necessary, a direct constructive dialogue (discussions based on the educational goals and methods of each course) is held to reach a consensus and decisions are made and implemented through mutual understanding. For constructive dialogues to be held in person, if students prefer coordination by the Faculty of Agriculture in Form 1, the students should request the Student Affairs Section, and the Student Affairs Section will coordinate the implementation method, schedule, etc., be present, and record the content of the dialogues.
8. Implementation of Consideration
The teacher in charge will discuss with the Student Affairs Division any necessary preparations for the implementation of consideration. The faculty member in charge of the student will form a consensus and implement consideration based on the constructive dialogue described in 7. above and the consultation described in 11.
9. Report and consultation on cases that cannot be handled by the Faculty of Agriculture alone.
The dean consults with the general supervisor (Director in charge of promoting support for students with disabilities. The Student Support Division is the contact point).
The general supervisor gives instructions, etc. to the Student Support Division. The Student Support Division will coordinate with the Finance Department, Facilities Department, etc., and adjust, etc., for the implementation of considerations. The general supervisor will submit the matter to the Special Committee for the Promotion of Support for Persons with Disabilities to discuss the measures to be taken, and then notify the Dean of the Faculty of the contents of consideration decided upon.
10. Monitoring of the contents of consideration.
Monitoring is conducted by the Special Committee for the Promotion of Support for Persons with Disabilities using the information on the e-portfolio.
11. Appeals
If a student is dissatisfied with the consideration decided by the dean, he/she may appeal to the person in charge of general supervision (the director in charge of recommending support for persons with disabilities, with the Student Support Division as the contact point).
12. Others
Students may consult with the Student Support Division if there is a delay in the decision on or implementation of consideration despite the submission of a written request for reasonable accommodation, or if there are other matters that require consultation in response to the request.
The Support Section for Inclusion will provide consultation as appropriate.
